NEXT GENERATION POPULATION HEALTH: Demonstrating a comprehensive approach to risk prediction and prevention for cardiovascular disease (CVD) patients.


The "Next Generation Population Health" project is a collaborative effort between Hywel Dda University Health Board, Swansea Bay University Health Board, Swansea University, and Amgen, aimed at improving cardiovascular disease (CVD) risk prediction and prevention in Wales.

The project consists of two phases:

  • Phase 1 involved analysing trends and gaps in CVD and lipid management using the SAIL databank, revealing an increasing prevalence of CVD, suboptimal lipid management, and the negative impact of COVID-19 on testing frequency.
  • Phase 2 focused on implementing and evaluating high-risk CVD management clinics led by specialist pharmacists, despite facing delays due to poor primary care engagement and information governance issues.

The initial feedback from the clinics, which aimed to optimise lipid and blood pressure control, was very positive, and the evaluation will continue until early 2025.
